Excess weight can have a significant impact on the physical and mental wellbeing of a person. Even small reductions in body weight may reduce the risks of many chronic conditions. However, if it was as easy as “Eat less, move more,” we wouldn’t be in the middle of a global obesity epidemic. The fact is that our bodies are designed to defend weight and there are a whole host of genetic and hormonal factors that make it difficult to lose weight and even harder to maintain weight loss.
 
Dr Keshinie knows that most patients would have tried several different strategies to lose weight by the time they come to see her. She spends time taking a detailed history to understand your weight journey and the impact it has had. She wants to know you as a whole person, not just a list of your medical conditions, and will ask you about sleep, stress levels, work, family and any other current priorities as these will all impact your management plan.

Nutrition and Physical Activity

Dr Keshinie uses her knowledge of Lifestyle Medicine and Weight Management to assess your current levels of nutrition and physical activity. While optimising these areas alone only confer a smaller percentage of body weight loss, they still form the foundation of good health and wellbeing. For some patients there may not be many changes to make in these areas and for others one or both may be areas of focus in the management plan. Dr Keshinie believes in arming you with up-to-date evidence-based advice and then working with you to develop an individualised plan tailored to your goals and life circumstances. As an Accredited Health Coach, she has the tools to help you harness your own strengths to make and sustain positive behavioural change.

Sleep and Stress

These often overlooked areas are pivotal in weight management and may be the barrier holding you back. Dr Keshinie’s training in Cognitive Behavioural Therapy and Insomnia Management allows her to provide you with strategies in these domains.

Very Low Energy Diets/Low Energy Diets

Dr Keshinie believes in long term sustainable behavioural changes. However, sometimes a kick start with meal replacements may be beneficial. As an Optifast Accredited Healthcare Professional, she has the knowledge to provide medical supervision of these diets and can work with you to return to a maintenance diet with a reduced risk of weight regain.

Medications

There are now a variety of medications indicated for the treatment of excess weight. They are available for patients with a BMI>30 or those with weight related medical conditions and assist in reducing appetite, increasing fullness and reducing cravings. They counteract the body’s natural propensity to conserve weight and work best as an adjunct to healthy nutrition and physical activity. Just like medications for other chronic conditions, they are designed to be taken long term to prevent the risk of weight regain. When prescribing medications, Dr Keshinie will have a comprehensive discussion with you about the available options and their potential side effects. She will help adjust the dose based on your response to the medications and will review you regularly especially in the initial few months until the dose stabilises.

Surgery

For patients with medical complications related to their weight, or a BMI >35, surgical options are also available. Dr Keshinie can talk you through the available options to see if any of these are right for you. She can assist in the referral to an appropriate surgeon and can support you before and after the procedure.
